Stuck in Drafts Prompt Action purgatory

I am appealing to those of you who are Drafts app geniuses.

I have a running list of my weight and have created an action in Drafts to prepend a new entry to the top of the list (this works successfully for anything I write in a blank draft and then run the action titled “Add to Draft”).

I would like to run this action from the Drafts widget with a prompt for a new top line entry (ie: skipping opening a new draft, typing an entry, and then running the action). For some reason, I get the prompt to “complete” but it does not prepend any data. I have tried every configuration I can think of (including the use of “[[draft]]”. What would you advise? In common language, how would you set this up? Screenshots attached. I hope this is clear. TIA.
