Help please with my simple Reminders Shortcut

I recently posted why I couldn’t do certain things with Apple Notes and Reminders apps.

I have, however, been using an Apple Reminders shortcut to replicate the Quick Entry feature of OF and Things. However, I can only get the first part of the shortcut to work. I want the second set of the shortcut fields to show up when I activate the Reminders shortcut.

Below are two screenshots. The first shows the shortcut. The second shows what I get when I activate it. As you can see, only the first part of the Reminders shortcut activates.

What am I doing wrong?

I can only test on iOS (I don’t have Ventura) but you can set most of those fields to “Ask every time”. Not sure about fields like flag, though.
