Running macOS Sonoma
The sensitivity of the microphone always goes down and I can’t get it stick to maximum sensitivity.
People can’t hear me when I make a call and I have to crank it up every time I start.
In the past we could set it to auto or select manually the input level.
Is it just me or a bug?

This is a different problem, but it might have some hints where the problem lies (are you using any Yealink software? Does this happen in every type of call — Zoom, Teams, FaceTime, etc.?) Or possibly it’s the same issue and the auto-leveling is your problem.

I am not using the Yeahlink software. And yes it happening in every application what needs the microphone. Doesn’t matter which one.
So yes the problem is in my macOS.

If none of the Stack Exchange stuff helps, you might try deleting the plist files for audio. It’s possible that a corrupt file is stopping System Settings from remembering the correct input volume.
I also wonder if this behavior is due to noise cancelling? Sometimes there can be problems when multiple layers of noise cancelling interfere. Sonoma added a feature called “voice isolation”, and a quick web search shows that some manufacturers recommend turning this feature off. That means “mic mode” should be set to standard, not voice isolation. See this:
