Anyone else using the standard podcast app and it randomly download old (like 2-3 years old) episodes? Very weird and I don’t see a pattern. I just mark them played, but it is somewhat annoying.

I use PocketCasts. I can’t stand the stock Podcast app.
Honestly Apple should just focus on the system and core apps like Notes, Reminders, etc.

I’ve seen podcast feeds that switch providers which changes the url and can trigger a download.

That’s based on changes in GUID. If the ID changes, apps see it as a new episode, even if all other metadata matches. In our CMS, we basically hardcode the GUID so it can’t easily be changed. My money is that Apple Podcasts is losing track of episode status. I would think that unsubscribing and then resubscribing would clear it up.
